The answer is D. Venography
Venography is the historical gold standard for diagnosing DVT. Also called contrast phlebography, this invasive procedure involves injecting radiopaque contrast into a dorsal foot vein followed by X-ray imaging to visualize filling defects indicating thrombi. Venography offers superior sensitivity (95-100%) and specificity (90-100%) for detecting DVTs throughout the venous system, including isolated calf vein thrombi. Despite this diagnostic accuracy, venography has been largely replaced by non-invasive alternatives in clinical practice due to its invasiveness, cost, technical difficulty, and potential complications including contrast reactions, pain, and paradoxically, DVT induction. For board exams, it’s critical to recognize venography as the gold standard while understanding it’s now typically reserved for situations where other diagnostic methods are inconclusive.
Incorrect Answer Explanations:
- D-dimer is a fibrin degradation product elevated when clots are being actively broken down. While highly sensitive (>95%), it has poor specificity (~40%) as it’s elevated in numerous conditions (inflammation, cancer, pregnancy, recent surgery). D-dimer is valuable for ruling out DVT in low-risk patients when negative, but cannot diagnose DVT when positive.
- Venous duplex ultrasound is the current first-line diagnostic test for DVT in clinical practice. It combines B-mode ultrasound with Doppler assessment of venous flow. While highly accurate for proximal DVTs (sensitivity >95%, specificity >95%), it has limitations for calf veins, iliac veins, and in obese patients. It’s not the gold standard despite being the most commonly used test.
- MRI can visualize thrombi directly and provides excellent anatomic detail. However, it has limited availability, higher cost, longer acquisition times, and contraindications (metallic implants, claustrophobia). While useful in certain scenarios, it’s not considered the gold standard for DVT.
- CT angiogram (CTA) is primarily used for diagnosing pulmonary embolism rather than DVT. While CT venography can be performed concurrently with pulmonary CTA, it exposes patients to radiation and contrast, and isn’t routinely used for isolated DVT evaluation.
