Pemphigus vulgaris is a life-threatening dermatologic disorder characterized by blister formation in the skin and mucous membranes. It is classified as a Type II hypersensitivity reaction. Precipitating factors include dietary changes, UV radiation, viral infections, and certain medications (e.g., captopril or penicillamine). Clinical features may include painful, flaccid blisters (commonly with mucosal involvement), erosions, and crusts. The Nikolsky sign (mechanical pressure at the edge of blisters leads to induction of blistering and sloughing off of epidermis/skin) will be positive. Diagnosis is confirmed with a biopsy. Rituximab combined with systemic corticosteroids is now the most appropriate first-line treatment for moderate-to-severe pemphigus vulgaris.
