A drug-induced phytotoxic reaction is essentially a photosensitive response to a drug metabolite. Phototoxic reactions occur when ultraviolet or visible light activates a photosensitizing drug, resulting in cellular injury that manifests as a sunburn-like reaction in sun-exposed areas. The reaction can occur on first exposure to the combination of drug and light, does not require prior sensitization, and may produce vesicles or blisters. Most common causes include tetracyclines, thiazides, fluoroquinolones, NSAIDs, amiodarone, etc. Clinical features may include severe sunburns and rashes with vesicles. Treatment is sunscreen and discontinuing the drug (if possible).
