An incompetent cervix or cervical insufficiency is defined as cervical dilation in the absence of uterine contractions or labor, typically owing to a functional or structural defect. The most common acquired cause is cervical trauma secondary to LEEP, conization, or cervical lacerations during previous childbirth. The diagnosis is often made retrospectively, as the patient may have a history of 2nd-trimester loss. Many do not have symptoms, but they could have vagina discharge, pelvic pressure, and spotting. The definitive surgical treatment is cervical cerclage, which is suturing of the cervix to prevent it from dilating prematurely.
