Widespread pain index (WPI) and symptom severity (SS) scale scores are part of the American College of Rheumatology’s alternative criteria for the diagnosis of fibromyalgia. These criteria focus on the extent of pain and the severity of symptoms over time, rather than on physical examination findings alone. The WPI assesses the number of painful areas out of 19 specified body regions, and the SS scale evaluates the severity of fatigue, waking unrefreshed, cognitive symptoms, and the extent of somatic symptoms in general. To diagnose fibromyalgia, the widespread pain index must be greater than seven, and the symptom severity scale must be greater than five for at least three months.
