Osteoarthritis is defined by progressive loss of articular cartilage with reactive changes at joint margins and in the subchondral bone. It is the most common joint disease in the United States and one of the leading causes of disability in those older than 65 years old. Risk factors include age, obesity, trauma/infection, and being female. Clinical features include joint pain and stiffness. On a physical exam you may see Heberden nodes (of DIPs) or Bouchard nodes (of PIPs), decreased ROM, and mechanical symptoms (clicking or locking). Diagnosis includes x-rays. Management includes weight management, physical therapy, NSAIDs for pain/inflammation. Surgery (e.g., total knee arthroplasty) can also be considered for knee osteoarthritis.
