Pemphigus Vulgaris is a life-threatening dermatologic disorder characterized by blister formation in the skin and mucous membranes. It is classified as a Type II hypersensitivity reaction. Precipitating factors include diet changes, UV radiation, viral infection, and drugs (e.g., captopril or penicillamine). Clinical features may include painful, flaccid blisters (commonly with mucosal involvement), erosions, and crusts. The Nikolsky sign (mechanical pressure at the edge of blisters leads to induction of blistering and sloughing off of epidermis/skin) will be positive. Diagnosis is confirmed with a biopsy. The first line is high-dose systemic steroids.
