Peyronie’s disease (PD) is a localized fibrotic disorder of the tunica albuginea that occurs in about 5% of males. Simply put, fibrosis scar tissue develops inside the penis and leads to curved, painful erections (+/- erectile dysfunction). The cause is multifactorial (genetics, trauma, tissue ischemia). Risk factors include genital/perineal injuries, urethral instrumentation, older age, underlying connective tissue disorder, Paget disease, gout, smoking, diabetes, etc. About 20% of patients with PD also have Duputreyn’s contraction. Diagnosis is usually apparent from patient history and examination. A urology referral is recommended if the condition is suspected. Management options include observation, medical therapy (e.g., pentoxifylline or intralesional collagenase Clostridium histolyticum injections), or surgery, depending upon the severity of the disease.
