Rh incompatibility is serious as it can lead to hydrops fetalis & hemolytic disease of the newborn. Rh incompatibility occurs when the mother’s blood type is Rh negative and her fetus’ blood type is Rh positive. This becomes clinically significant if a mother (who is RhD-negative) ends up producing anti-D antibodies that can potentially destroy RhD-positive erythrocytes of the fetus. The USPSTF recommends Rh(D) blood type and antibody screen for all pregnant women at the initial prenatal visit. They recommend repeat testing for all unsensitized Rh(D)-negative women at 24 to 28 weeks gestation.
