Croup is an upper respiratory condition with inflammation in the larynx and subglottic airway, usually caused by Parainfluenza virus. Other etiologies include RSV, adenovirus, COVID-19, measles, and, rarely, bacteria. It is common in children < 6 years old. Clinical features include fever, barking cough, stridor, hoarseness, drooling, throat pain, and difficulty swallowing. Classically, on x-ray, you may see the “steeple sign, which is subglottic narrowing. Diagnosis is clinical. Management of mild croup includes supportive care (e.g., mist, antipyretic, and fluid) plus a dose of steroids. Those with moderate to severe croup should be given steroids, nebulizer epinephrine, fluids, and oxygen.
