Esophageal Cancer: The Daily PANCE Blueprint

A 63-year-old man with a long history of GERD and a recent diagnosis of Barrett's esophagus presents with progressive difficulty swallowing solid foods. Which type of esophageal cancer is he most at risk for?

A. Small cell carcinoma
B. Squamous cell carcinoma
C. Adenocarcinoma
D. Lymphoma
E. None of the above

Answer and topic summary

The answer is C. Adenocarcinoma

Adenocarcinoma and squamous cell carcinoma (SCC) account for ~95% of esophageal cancers. The most common risk factors for SCC include smoking and alcohol; this is different from adenocarcinoma which is associated with Barrett’s esophagus due to gastroesophageal reflux disease. Clinical features of esophageal carcinoma include weight loss, progressive dysphagia, anemia, and low iron. Diagnosis is made by upper endoscopy with biopsy. Treatment depends on the severity/type.
