In a young, sexually active male presenting with dysuria, urethral pruritus, and discharge, infectious urethritis is the most likely diagnosis. The two most common etiologic agents for urethritis in this demographic are Chlamydia trachomatis (non-gonococcal urethritis) and Neisseria gonorrhoeae (gonococcal urethritis).

While gonococcal urethritis classically presents with a more purulent discharge and chlamydia with a clearer, mucoid discharge, both must be suspected, tested for (typically via NAAT), and often treated empirically together.
