The organism is a fungus that was long misclassified as a protozoan — which is why it is treated with an antimicrobial rather than a conventional antifungal, and why the older name P. carinii still appears in the abbreviation PCP. The CD4 threshold of 200 cells/µL is the number to know: it defines the risk period, it triggers prophylaxis, and it should be the first thing you look for in any HIV stem. The clinical signature is the mismatch between how sick the lungs are and how normal they sound. The infection fills alveoli with a foamy exudate and thickens the interstitium, impairing gas exchange diffusely without producing the consolidation that generates crackles and bronchial breath sounds. That is why the examination is clean while the patient desaturates walking — and why ambulatory pulse oximetry is such a useful bedside test, since a resting saturation can be entirely reassuring. Radiographically the pattern is diffuse bilateral perihilar or “bat-wing” interstitial infiltrates; high-resolution CT shows ground-glass opacities and is more sensitive when the film looks normal. The highest-yield management point is the steroid rule: add adjunctive corticosteroids when the room-air PaO2 is below 70 mm Hg or the alveolar-arterial gradient exceeds 35 mm Hg, because treatment kills organisms and releases antigen, provoking an inflammatory surge that can worsen hypoxemia — steroids blunt it and reduce mortality. Prophylaxis with TMP-SMX begins at a CD4 below 200 and continues until the count is sustained above 200 for three months on antiretroviral therapy.
