Ectropion: The Daily PANCE Blueprint

A 72-year-old male presents to your clinic concerned about his right eye. He says his eyelid keeps “turning out." On physical exam, you notice his right lower eyelid drooping away from his eye and outward turning of the right lower eyelid margin. What is this called?

A. Ectropion
B. Entropion
C. Retinal detachment
D. Strabismus
E. Conjunctivitis

Answer and topic summary

The answer is A. Ectropion

The patient has ectropion, which is the outward turning of the eyelid margin. It is very common in older adults and occurs due to the weakening of muscles over time. The inner eyelid and conjunctiva can be exposed, red, and irritated. Patients may have excessively watery eyes, gritty/sandy eyes, sensitivity to light, etc. Management, in this case, includes artifice tears and lubricating ointments. Surgery is the definitive treatment though. If left untreated, a patient is at an increased risk for eye infections and corneal damage.
