Extensive impetigo or ecthyma (deep impetigo) is best treated with oral antibiotics, particularly in cases with numerous lesions. Cephalexin (500 mg QID) and dicloxacillin (500 mg QID) are first-line oral options for impetigo due to their effectiveness against Staphylococcus aureus and Streptococcus pyogenes. A 7-day course is typically sufficient. For methicillin-resistant Staphylococcus aureus (MRSA), trimethoprim-sulfamethoxazole, clindamycin, or doxycycline is preferred.
