Otitis media is defined by inflammation of the medial ear, it can be acute or chronic. Acute otitis media (AOM) is common in children; in fact, about 80% of children have had at least 1 episode of AOM by age 3. Etiologies include bacterial (most commonly S. pneumoniae) or viral (e.g., RSV, influenza, rhinovirus, and adenovirus). There is a strong genetic component. Risk factors include age, routine daycare, low SES, smoke exposure, and underlying ENT disease. Clinical features include otalgia, decreased eardrum mobility, bulging of the tympanic membrane, otorrhea, restlessness, poor appetite, and tugging at the ear. The preferred initial treatment is amoxicillin. Alternatives include Augmentin, cefdinir, azithromycin, etc.
