Patent Ductus Arteriosus (PDA) occurs when the ductus arteriosus fails to close after birth, creating a left-to-right shunt with increased pulmonary blood flow.
Indomethacin is a non-selective NSAID that inhibits cyclooxygenase (COX) enzymes, decreasing prostaglandin synthesis. Since prostaglandins maintain ductal patency, reducing prostaglandin levels promotes ductal closure. Administered intravenously in three doses over 24-48 hours with 70-80% success rates in premature infants. Most effective when given within the first week of life.
Clinical Pearls:
- Indomethacin CLOSES the PDA; Prostaglandin E1 KEEPS it OPEN – remember “Indo = In-Close”
- Contraindications to indomethacin: renal dysfunction, thrombocytopenia, bleeding, NEC
- Monitor during treatment: renal function, urine output, platelets, GI tolerance
- Alternative: Ibuprofen has fewer renal side effects
- If medical management fails: consider repeat indomethacin course or surgical ligation
