Premature atrial contractions (PACs) and premature ventricular contractions (PVCs) most commonly present with palpitations, described as isolated, skipped, or strong heartbeats. PACs and PVCs can occur as single beats or in complex patterns such as bigeminy or trigeminy. Symptoms may include fatigue, dyspnea, dizziness, chest pain, and presyncope. The diagnostic evaluation begins with a 12-lead ECG, followed by ambulatory monitoring to assess ectopy burden. Increasing PAC burden is associated with the development of atrial fibrillation, while increasing PVC burden can lead to PVC-induced cardiomyopathy and heart failure. PACs are associated with age, left atrial enlargement, and intraventricular conduction blocks. PVCs are associated with age, lower HDL, elevated BNP, LV hypertrophy, and reduced LVEF. PACs are usually benign and often require only reassurance. If someone has symptomatic PACs, then beta blockers are the first-line pharmacologic treatment. The management of PVCs depends on symptoms, PVC burden, and presence of left ventricular dysfunction.
