Chronic bacterial prostatitis is a condition defined by chronic or recurrent urogenital symptoms with concurrent evidence of bacterial infection of the prostate. The most common cause is gram-negative rods (Escherichia coli in particular causes 75 to 80 percent of episodes). Clinical features may include urinary frequency or urgency, perineal discomfort, low-grade fever, bladder irritation, persistent bacteriuria, prostatic hypertrophy or tenderness, etc. It’s important to note that many people are asymptomatic, and prostate exams may be normal. The treatment of choice is prolonged antibiotic therapy (4-6 weeks); first line is ciprofloxacin or levofloxacin. Alternative agents include: trimethoprim-sulfamethoxazole, macrolide, doxycycline, oral beta-lactams.
