Celiac disease is an autoimmune disorder defined by sensitivity to gluten. It is more common in patients with T1DM, autoimmune thyroiditis, Trisomy 21, and Turner syndrome. Clinical features include diarrhea with bulky, foul-smelling floating stool, flatulence, weight loss, anemia, and atrophic glossitis. Patients may have dermatitis herpetiformis, which is a pruritic rash typically located on the elbows, forearms, and knees.
Laboratory studies to order include anti-tissue transglutaminase antibodies, anti-endomysial antibodies, and antigliadin antibody assays. The best diagnostic test is an upper endoscopy with a biopsy. Patients must avoid food with gluten, which includes barley, rye, oats, and wheat (BROW).
