A 42-year-old male with a history of hypertension, diabetes, hypothyroidism, and tobacco abuse presents to the ER due to severe tooth pain. He is febrile (T 101.3) and tachycardic (HR 104 bpm). On physical exam you note what appears to be a periodical abscess with purulent drainage from the gum line. Which of the following is the best overall management for this patient?
A. Obtain CT head/neck, obtain cultures, give a fluid bolus, start IV amoxicillin / clavulanic acid, and consult an oral surgeon
B. Start IV piperacillin/tazobactam, obtain blood cultures, and start IV fluids
C. Give acetaminophen, order a 30 cc/ kg fluid bolus, and send home with antibiotics
D. Send the patient home with PO antibiotic and have the patient follow-up with a dentist soon
E. Obtain CT chest, CT head/neck, and start the patient on piperacillin/tazobactam
